What is Dementia?

Dementia is characterized by a loss of memory and the inability to think clearly and process thoughts. In our Brain Longevity Center, we diagnose and treat various causes of dementia with a focus on Alzheimer’s dementia, which accounts for approximately 60-80 percent of all dementia.

Alzheimer’s disease is thought to be directly linked to the number and density of entangled neurons surrounded by abnormally thick and sticky proteins. This entanglement is known as a plaque. The plaque decreases, if not completely hinders, the neurons' ability to communicate effectively with one another. The plaques are located in the hippocampus, which is responsible for our memory.

Is there a cure for Dementia? Can one regain their memory capabilities? 

Once one is diagnosed with Dementia, the journey of decreasing mental capacity begins. You can only hope that the rate of declining mental capacity can be slowed. The symptoms only get worse; typically, the person, over time, becomes a shadow of themselves. In the final stages of Dementia, the patient needs to be assisted with the most basic activities of daily living.  Patients cannot be left alone at home; their behavior can be erratic, unpredictable, and unsafe.  Even a routine walk around a familiar neighborhood can lead to getting lost. 

Recent Pharmaceutical Breakthroughs 

Within the past two years, new drugs classified as monoclonal antibodies given intravenously have been shown to significantly decrease the rate of mental decline in Alzheimer’s dementia. Clinical studies show that the treatment administered in the early stages of the disease leads to the best outcomes, which is defined as slowing the rate of mental decline. It is now known that lesions in the brain or plaques can be seen decades before a patient presents with symptoms.  It is the unique plasticity capability of the brain to “ re-wire” around the plaques, which can explain why the symptoms of decreased memory and cognitive abilities are not clinically observed for many years.

We think patients must seek neurological consultation as soon as a change in memory capacity is detected.  We want to change the perception that a slight loss of memory is not just part of “getting old” but an interventional opportunity to address developing neuronal plaques. 

Diagnose early, efficiently, and quickly.

We are vigilant in using every possible diagnostic tool available to diagnose dementia accurately.  Our approach includes:

A) In-depth Cognitive testing of various memory capabilities in conjunction with measuring thought processing. 

B) Blood test to detect abnormal proteins, (Lumipulse G pTau217/B Amyloid 1-42 Plasma Ratio) 

C) Genetic Test, to determine if one carries 1 or 2 genes for APOE-e4

D) EEGs to measure and observe specific patterns of brain waves. Brain waves are less complex and changes in EEG synchrony  

E) Radiology Diagnostic Testing. We have long-standing relationships with academic diagnostic radiology centers to evaluate and discover the earliest signs of dementia using PET Scans with an amyloid ligand tracer

F) Cerebrospinal fluid analysis of measuring levels of abnormal Tau and amyloid proteins. The procedure is performed in a comfortable setting with anesthesia.
